Please note that transcript release forms may be submitted in one of three ways:

• mail to HVCHS, Attn: Counseling Services, Mrs. Valcarcel, 259 Pennington-Titusville Rd, Pennington, NJ 08534

A signed Transcript Release Form must be submitted (by parent if student is under 18 years of age) in order for HVCHS to send a transcript.

During senior year only, students applying to college must complete and submit one Senior Transcript Release Form which will authorize HVCHS to send an official transcript to any college or scholarship program upon request.  Then, in order to have a transcript sent to a specific college or scholarship program, a completed Senior Transcript Request Form (also available in the Counseling office as a pink card) must be submitted for each individual transcript requested (i.e. - one completed form or pink card is required for each college).  The Senior Transcript Request Form does not require a parent signature.

The Student Brag Sheet and Parent Brag Sheet will assist your counselor in writing an informed and effective recommendation for you.  Although you can print the pdf form, complete it and return it to your counselor, or save the Word version to your desktop, complete it and email it back to your counselor, we would much prefer that you complete these sheets online by logging into Family Connection.  The Academic Teacher Brag Sheet will assist your teacher in writing an effective teacher recommendation for you, one that focuses on your classroom performance.
